The USCCB Vulnerable Care Services program provides vulnerable newcomer populations access to care and services through the delivery of case management in areas where ORR-funded intensive case management is not available or at capacity.

The scope of this program includes but is not limited to:

Identifying vulnerable clients in need of intensive case management and referring them to USCCB for enrollment consideration.

Conducting intake and needs assessment and developing service plans.

Providing intensive case management services and monitoring client progress.

Submitting monthly case management reports and requests for reimbursement.

Maintaining case files.

Submitting required quarterly reports.

Maintaining relationships with other community services providers to ensure VCS clients are included and have access to their services

Ensure there is sufficient capacity for capacity building to support vulnerable refugees. This includes volunteer coordination, outreach, and community engagement activities.
