VCS

The USCCB Vulnerable Care Services program provides vulnerable newcomer populations access to care and services through the delivery of case management in areas where ORR-funded intensive case management is not available or at capacity.

The scope of this program includes but is not limited to:

​

  • Identifying vulnerable clients in need of intensive case management and referring them to USCCB for enrollment consideration.

  • Conducting intake and needs assessment and developing service plans.

  • Providing intensive case management services and monitoring client progress.

  • Submitting monthly case management reports and requests for reimbursement.

  • Maintaining case files.

  • Submitting required quarterly reports.

  • Maintaining relationships with other community services providers to ensure VCS clients are included and have access to their services

  • Ensure there is sufficient capacity for capacity building to support vulnerable refugees. This includes volunteer coordination, outreach, and community engagement activities.
