WHAT WE DO

The PC program provides intensive case management for refugees and other vulnerable individuals who have been in the United States for less than five years and need additional support integrating into their new communities.

The PC Program offers comprehensive assistance to help clients overcome barriers to stability and independence.
 

Our services include:

  • Intensive case management and ongoing support

  • Social services assistance and referrals

  • Disability support and medical assistance coordination

  • Employment readiness and job search coaching

  • Guidance on understanding workplace culture and opportunities

Please note that the clients in this program do not receive cash allowance. All services are provided through case management and direct support connections, ensuring resources are used efficiently yo meet each client's needs. 

WHO WE 
SERVE

The PC Program primarily serves:

  • Refugees and humanitarian entrants who have lived in the U.S. for less than five years

  • Individuals without family or social support systems

  • Elderly refugees (65+), individuals with disabilities, or those facing complex health and social needs

Each client is paired with a dedicated case manager who provides individualized support and guidance throughout their journey.

PROGRAM GOALS

Our focus is to empower clients to:

  • Maintain stable housing and access essential benefits

  • Improve health and wellness through medical and community resources

  • Develop job readiness and sustain long-term employment

  • Integrate fully into their local communities

  • Build confidence and independence in everyday life
